Ela Soccer Club

All goals must be securely anchored when in use. A physical inspection of the goals must take place before use in a game or practice. The Goal Inspection Form must be completed by the responsible coach or parent and returned to the Director of Safety at the end of the season.

All coaches and volunteers must complete a background check. All Head and Assistant coaches must fill out the IYSA Disclosure Form  prior to the season start. Completed forms must be faxed or sent to Jan Janus.

In order to promote sportsmanship please ensure the Player Rating Worksheet is returned to the office promptly at the end of the season

The Ela Soccer Club would like to use pictures of its players in the Club’s promotional materials and put team and individual photos on its web site. The Ela Soccer Club will not disclose names of any players pictured on any of its promotional material or website. Please grant permission to the Club. Please distribute the Photograph Release Form to your player’s parents. Completed forms can be faxed or sent to the office.
